Possible causes

Partially clogged DPF filter
Failed back pressure sensor
Excessive soot accumulation
Physical restriction in the exhaust

Recommended actions

Perform forced regeneration
Check pressure sensor hoses
Inspect muffler and exhaust outlet
Clean or replace DPF filter
